WNC Staff Exit Form

When your department's staff or student employees leave your department or the university, please fill out this form.

WNC will remove the employee's access at the date and time requested.

Should we leave the computer as-is for the next user, or wipe the computer and rebuild it?
Should we move the employee's network data to a defined space on the shared drive? For example, move everything from users\wilburwildcat to users\_former employees\wilburwildcat.
Should we copy the employee's data under their user folder to a defined space on the shared drive? For example, move everything from their local documents and desktop to a shared space under something like users\_former employees\wilburwildcat.
Which Departmental Account (s) should the employee be removed from?
Retirees
UA email belongs to the retiring employee. If staff email data will be needed post-exit, the department and the employee will need to negotiate terms of access.
Students
UA email belongs to former and continuing students. If student worker email data will be needed post-exit, the department and the student worker will need to negotiate terms of access. If an agreement cannot be reached and the department needs the former student worker's email to be forwarded or backed up, a written request from the department head needs to be given to WNC on departmental letterhead.
Staff Terminations
If an agreement cannot be reached and the department needs the former employee’s email to be forwarded or backed up, a written request from the department head needs to be given to WNC on departmental letterhead.
