WNC New Staff Hire Form

Please fill out this form as soon as possible after a new staff or student has been hired, so WNC can prepare/order a computer. This process can take several weeks.

A NetID is required for many of the steps to get a new employee set up in the system, however, if a computer needs to be ordered and you don't yet have a NetID, please complete this form anyway.

WNC will need at LEAST 4 business days’ notice prior to the start date of the employee in order to configure the computer account.

Employee Information
Provide information for the new staff or student being hired.
(if available) Employees not previously employed by the University of Arizona, will need to have a NetID created, for more information visit: netid.arizona.edu.

Who should this person have permissions like, if applicable? For example, "This person should have the same access as Sam Wilson."
Computer
Which computer will they be using? e.g. Jill's computer, or SPS128
Calendar Settings
Who will need to share their calendar with this new employee? Please list names and/or email addresses. If they are to be granted access to other calendars, access will need to be granted from the individual staff members.
Who should this new employee share their calendar with? Please include names and/or email addresses.

Department Specific Items

Certain departments have unique requirements, please complete this area if you do.

Which Departmental Account(s) should this employee have access to? Please also include if they should have Full Mailbox rights or not; and whether they should have Send-As or Send-On-Behalf-Of rights for each account listed.
TLA
*Not available for student workers Is the person's name formatted correctly in EDS so they can have the @tla email address they desire? For example, if EDS shows the person's preferred name as Wilbur E. Wildcat, the @tla email address will become WilburEW@tla.arizona.edu. Once the preferred name is set (e.g. Wilbur Wildcat), then when we apply policy, the email address will become WilburW@tla.arizona.edu.
Choose which Group(s) the employee should belong to
Will the employee need access to special areas, such as Finance or Wheelhouse folder(s)? Please specifiy.

Department Responsibility: If the student needs a Catworks Email and/or Lync account, please go to Request Catworks Account and request the accounts. Keep in mind that Catworks account are dependent on HR classifications and may take longer to process.
