We recommend that before you complete this form you review your current listings in the hard copy of the business white pages directory (primarily pages 98-100) or online at DexPages.
If changes are required, indicate the change, addition, or deletion on your current listing(s). All changes, including additions and deletions require your name, phone number, and email address. In ALL cases where changes are required, we ask that you email a copy of your current listing(s). Failure to do this may result in errors.
